The certificate states that Oberkanonier (Senior Gunner) Ernst Heller of Artillery Regiment 4 was wounded on November 29, 1943 and has been awarded the Wound Badge in Black. The document is dated to Goldberg (modern day Złotoryja, southwest Poland) on December 22, 1943 and signed in black ink by a Stabsarzt and Chief Medical Officer, the signature is indecipherable.
