In silver, obverse illustrating the Order of St. John Maltese Cross surmounted by a dragon, framed by a ribbon banner below inscribed "MIDLAND RAILWAY AMBULANCE", maker marked "AHD" to the left of the cross, marked with an anchor (made in Birmingham), the British lion and date marked "e" (1904) to the right of the cross, reverse with raised text and personalized engravings, inscribed "AWARDED TO JAS DAY FOR FIRST AID RENDERED AT BRISTOL FEB 1st 1904.", measuring 31.7 mm in diameter, ring suspension, extremely fine.
