Instituted in 1888. Neck Badge: in silver with white enamels, measuring 53.3 mm (w) x 56.3 mm (h), on its full-length neck ribbon; and Breast Star: in silver with white enamels, maker marked "V&S" (Vaughton & Sons, Birmingham) on the reverse, measuring 53.8 mm (w) x 53 mm (h), vertical pinback. Intact enamels, very light contact, near extremely fine. In their hardshelled case of issue, thatched-look on five sides on the exterior, pebbled on the bottom, inside lid in black satin and padded, with the Order of St. John insignia in silver-coloured ink in the centre, raised platform in black felt with recessed medal beds for the Badge and Breast Star, measuring 104 mm (w) x 208 (h) x 41.5 mm (d), scuffing evident on the top of the lid and on the bottom, case better than very fine.

It is awarded in five grades: Bailiff or Dame Grand Cross, Knight or Dame of Justice or of Grace, Commander Brother or Sister, Officer Brother or Sister, Serving Brother or Sister, Donat.
